With any physical activity program there is an element of risk. We need to let you know what the risks might be and have you acknowledge that you understand these risks. This form releases the University of Calgary from any liability for unintended issues that may arise as a result of your program or activity, due to circumstances we can't control.
1. Go to the Quick Links box, on the bottom left hand side of this page and choose Login to Account.
2. Login in using your UCID number. Your ID number is next to your name on your confirmation of registration. If this is your first time logging in, enter your ID number and the temporary password that was emailed to you. The system will prompt you to create a password, - login using your new password.
If you have not been emailed a temporary password please contact Campus Recreation at 403-220-7749.
3. Now click on My Events and view your registered programs. All courses you are registered in will appear, if a waiver is required it will say “outstanding”. Please click “sign waiver” button to sign the form.
** If you have any troubles with completing this waiver please DO NOT HESITATE to call Campus Recreation Client Services at 403-220-7749. **
To safely offer programs to you and your family, we need to be aware of any medical issues that you may have, as well as any life-threatening conditions, including, but not limited to severe allergies. We also need to know who to call in case of an emergency and contact information for your family physician.
1. Go to the Quick Links box, on the bottom left hand side of this page and choose Login to Account.
2. As the parent or guardian you must first login in using your own UCID number. If you don’t know what it is, you can phone Campus Recreation at 403-220-7749.
3.If this is your first time logging in, enter your ID number and the temporary password that was emailed to you. The system will prompt you to create a password. If you have not been emailed a temporary password please phone Campus Recreation at 403-220-7749.
4.After login you'll see the "My Profile" screen. While on “My Profile” scroll to the bottom of the screen to see a list of the family members associated with your ID number. If you do not see a list of family members, please phone Campus Recreation at 403-220-7749.
5.If a medical form needs to be completed or updated for you or a member of your family, click on the green "+" button, to the right of the persons name, then follow the instructions. You'll need the Alberta Health Care number for each family member you wish to create a medical form for.
6.If a medical form has already been completed, but you need to update the information, click on the blue box and update the required fields.
1. Once the medical form is added, or updated, you may need to complete a waiver (informed consent) form, for you or your child to participate in the program. To do this, begin by selecting “My Events” to view the registered events for you and/or members of your family.
2. Choose the family member registered in the program by using the drop down menu. All courses for that family member will appear, if an informed consent is required it will say “outstanding”.
3.Please click “sign waiver” button to sign the form.
4. If your family members do not appear in this drop down menu, please phone Campus Recreation at 403-220-7749.
